Appointment for signing a nationality declaration
Attribution of Belgian citizenship to a child under five years old
- Your valid Belgian passport or/and your valid Belgian identity card.
- Registration form for the child, completed and signed by both parents as well as a recent passport photo of the child.
- If you wish and if the child resides in the UK:
- An application form for a passport or Kids-ID card on the name of the child, completed and signed by both parents;
- as well as a prepaid self-addressed special delivery
- Your debit/credit card to pay consular fees
Declaration of Belgian nationality
- Your valid national passport and/or valid Belgian identity card.
- Your debit/credit card to pay consular fees.
- If needed any other documents specified by the consular department in a separate email
Appointment for signing a civil status certificate
- Your valid Belgian passport or/and your valid Belgian identity card.
- If one of the signatories resides in the UK: valid national passport and depending on the national status also a valid residence permit.
- Your debit/credit card to pay consular fees.
- Possible extra documents when signing a certificate of recognition
- Registration form for the child, completed and signed by both parents as well as a recent passport photo of the child
- If you wish and if the child resides in the UK: an application form for a passport or Kids ID card on the name of the child, completed and signed by both parents as well as a prepaid self-addressed special delivery.
